Sound Bath Participant Waiver and Release of Liability

Acknowledgment of Participation:
I voluntarily choose to participate in a sound bath session provided by Golden Hour Healing. I understand that the session is intended to promote relaxation and general well-being and may include the use of crystal or metal singing bowls, chimes, drums, voice, and other sound-producing instruments.

Health Disclosure:
I understand that sound bath sessions are educational and wellness-based experiences and are not medical treatment, mental health therapy, physical therapy, or a substitute for professional healthcare.

I agree to inform the Facilitator before the session if I have any condition that may affect my ability to participate safely, including but not limited to:

- Pregnancy
- Epilepsy or seizure disorders
- A pacemaker or implanted electronic device
- Severe tinnitus or sound sensitivity
- Vertigo
- Serious cardiovascular conditions
- PTSD or other conditions that may be triggered by sound or deep relaxation
- Any other medical condition for which my healthcare provider has advised caution
If I have concerns about whether participation is appropriate, I understand that it is my responsibility to consult my healthcare provider before attending.

Assumption of Risk:
I understand that participation may involve certain risks, including but not limited to:
- Emotional release
- Dizziness or lightheadedness
- Temporary ringing in the ears
- Discomfort related to sound or vibration
- Unexpected physical or emotional responses
I voluntarily assume all ordinary risks associated with participating. I understand that I may stop participating at any time and will notify the Facilitator immediately if I experience discomfort.

Release of Liability:
To the fullest extent permitted by applicable law, I voluntarily assume the risks associated with participating in this sound bath session and release the Facilitator, its owners, employees, contractors, volunteers, and venue from claims arising out of my voluntary participation, except where liability cannot legally be waived, including liability resulting from gross negligence or willful misconduct.

Photography:
Photographs and/or video that include me may be taken and used for educational or promotional purposes.

I certify that I am at least 18 years of age, or that a parent or legal guardian will remain present during the meditation.

I acknowledge that I have carefully read this Agreement, understand its contents, have had the opportunity to ask questions, and sign it voluntarily.
